HOW TO BE PREPARED FOR YOUR FIRST VISIT

• Complete your New Patient Paperwork in your PtEverywhere account under "Documents"

• Wear clothes you can move comfortably in

• Bring or wear a pair of sneakers with enclosed toes and heels - Recommended but not required

• You may bring copies of any recent imaging reports (Radiographs, MRI, CT scans) - however this is not required​​
